As with most things, there is a small amount of paperwork that must be completed prior to performing euthanasia. It ensures that you, as the owner or person who is legally duly authorized by the owner, understand what euthanasia is and gives consent to the attending veterinarian to euthanize your pet. In addition it asks if your pet has been exposed to rabies within the previous ten (10) days OR has bitten OR exposed any animal or person to rabies within the last ten (10) days (this may include deep scratches). (When is Rabies Testing Required?) Finally it states that you have chosen to take care of the aftercare process yourself (i.e. home burial).

Euthanasia and Cremation Forms

As with most things, there is a small amount of paperwork that must be completed.

The form ensures that you, as the owner or person who is legally duly authorized by the owner, understand what euthanasia is and gives consent to the attending veterinarian to euthanize your pet. In addition it asks if your pet has been exposed to rabies within the previous ten (10) days OR has bitten OR exposed any animal or person to rabies within the last ten (10) days (this may include deep scratches). In addition it explains cremation and the aftercare option that you have chosen: Communal or Private.  Compassionate Pet Vet acts as a facilitator between you and The Pet Loss Center of Austin for the cremation process (even if the attending veterinarian provides initial transport). Once transport has occurred, The Pet Loss Center of Austin is best suited to answer your questions regarding this process. However, if you should have any concerns that you wish to discuss with Compassionate Pet Vet, please do not hesitate to call….we are there for you.

If you would like to keep your pet’s collar, please do so prior to transport; once transported any personal items become the property of The Pet Loss Center of Austin.
