Euthanasia Consent Information
As with most things, there is a small amount of paperwork that must be completed prior to performing euthanasia. It ensures that you, as the owner, or person who is legally authorized by the owner, understand what euthanasia is and gives consent to the attending veterinarian to euthanize your pet.
In addition, it asks if your pet has been exposed to rabies within the previous ten (10) days OR has bitten OR exposed any animal or person to rabies within the last ten (10) days (this may include deep scratches). (When is Rabies Testing Required?) Finally it states that you have chosen to take care of the aftercare process yourself.

Euthanasia with Cremation Forms
As with most things, there is a small amount of paperwork that must be completed.
The form ensures that you, as the owner or person who is legally duly authorized by the owner, understand what euthanasia is and gives consent to the attending veterinarian to euthanize your pet. In addition it asks if your pet has been exposed to rabies within the previous ten (10) days OR has bitten OR exposed any animal or person to rabies within the last ten (10) days (this may include deep scratches). In addition it explains cremation and the aftercare option that you have chosen: Communal or Private. Compassionate Pet Vet acts as a facilitator between you and Fond Memories (formerly The Pet Loss Center of Austin) for the cremation process (even if the attending veterinarian provides initial transport). Once transport has occurred, Fond Memories (the Austin location) is best suited to answer your questions regarding this process. Items that are transported with your pet for cremation
All items that are transported with your pet to Fond Memories of Austin will stay with your pet until the cremation process. Items that can be cremated, per applicable regulations, will be cremated with your pet. All other items will be disposed of, they will not be returned to you. If you have any questions concerning what type of items can be cremated with your pet, please contact Fond Memories directly. All items transported by Fond Memories with your pet become the property of Fond Memories.
